Support at Home provides coordinated care and services to help you live independently at home.
There are 8 levels of support, ranging from ‘basic’ to ‘high-level’ care needs.
Support at Home can be used to purchase a range of care and services, including
- clinical care (such as nursing care and allied health)
- independence services (such as showering, transport and social support) and
- everyday living services (such as cleaning, gardening and meals).
The Support at Home Program also has three short-term pathways
- Assistive Technology and Home Modifications scheme – to help you get the home modifications and equipment that you need
- Restorative Care Pathway – to help you regain your independence after illness or injury and reduce the need for ongoing care.
- End-of-Life Pathway -to support comfort, dignity and choice at the end of life.
Your aged care assessment will determine which levels and types of support you are eligible for.
